123I-ioflupane (DaTSCAN®) SPECTなどによるdopamine transporter (DAT)の検出は、線条体のpresynaptic axon終末におけるDATを検出することで黒質-線条体路の神経変性の程度を描出するため、パーキンソン病の診断補助として広く用いられている[1]。パーキンソン症状が認められた場合、DAT SPECTで線条体における信号低下が認められれば、特発性パーキンソン病の他、多系統萎縮症、進行性核上性麻痺、大脳皮質基底核変性症、レビー小体型認知症などが鑑別にあがる。一方、信号低下がない例では、心因性、薬剤性/脳血管性のパーキンソニズムやジストニアなどの可能性も考えなければならない[1,2]。

しかしながら、大規模な多施設臨床研究ではパーキンソン病と診断された患者の3.6~19.6%はDAT SPECT正常であり、”SWEDD: scans without evidence of dopaminergic deficit”と呼ばれている[3]。SWEDDには、単純に診断が異なっている場合と早期のパーキンソン病である場合があると言われてきた[3]。早期のパーキンソン病であれば、ドパミン治療に反応し、数年後にパーキンソン病と同様の異常なDATイメージング所見を示すものもある[4]。一方、生前ドパミン反応性がありパーキンソン病とされていたが、剖検では黒質は正常で、線条体のみチロシン水酸化酵素低下が認められた症例も報告されている[5]。最近はドパミンではなく、例えばセロトニンシステムの異常と関連した病態の可能性を疑う研究者もいる[6]。またドパミン生合成に関与するpteridine代謝を阻害するGTP cyclohydrolase 1 (GCH-1) 欠損症は通常小児期に発症するドパミン反応性ジストニアとして知られているが、成人発症型のGCH-1 mutationはDATイメージング正常のパーキンソン病をきたすことが報告されている[7]。

このようにSWEDDは単一疾患ではなく、様々な病態が入り混じったものであり、”SWEDD”という言葉の意義は薄いとする考えもある[3]。
